Proof of Vaccine Deadline **reminder

Just a quick reminder that tomorrow
(Sept. 28th) at 10pm, is the deadline to bring your proof of at least one vaccine, or letter of exemption from OMHA, to the Shores Recreation Centre.If you can't get to the Shores Arena in Forest tomorrow between 4:30 - 10:00, you can email
a copy of the vaccine certificate to [email protected].
The proof of at least one vaccination is required for any player (born in 2009 and older), bench staff, volunteer (i.e., timekeeper), skills instructor, or board member.
Any of the above stated participants unable to provide proof of at least one vaccine, or letter of exemption, by Sept. 28th 2021, will be ineligible to play during the 2021-2022 season. This is in keeping with the OMHA’s timeline to complete full vaccination by Oct. 31st.
Refunds for any player withdrawing from LSMHA must be made in writing to [email protected]. Refunds will be granted according to the date the written application is received by LSMHA and the following table: **Note, there will be a $50 admin fee charged automatically for any refunds.
Up to September 30 - 90% of the remaining registration balance
During the month of October - 75% of the remaining registration balance
During the month of November - 50% of the remaining registration balance
During the month of December - 25% of the remaining registration balance
As of January 1 - 0% No late charges will be refunded.
