LSMHA Vaccination Update - UPDATED **
Hello Preds Families.  

Please see updates to this message (in red)

In an effort to keep families informed and up to date, LSMHA wanted to pass on new information related to the OMHA vaccination policy.  Earlier this week, the OMHA announced that any player aged 12 or over must be double vaccinated by October 31, 2021 in order to participate in minor hockey this season, and this includes Lambton Shores Minor Hockey.

Moving forward, the LSMHA will require, under the OMHA vaccination policy, that any player (born in 2009 and older), bench staff, volunteer (i.e., timekeeper), skills instructor, or board member show proof of at least one vaccine, or letter of exemption from OMHA, by 10pm on Tues. September 28th, 2021 at the Shores Recreation Centre.

Proof can be delivered to the registrar on the following dates:
Friday, Sept. 24th from 4:30-10pm  (Shores Arena)
Sunday, Sept. 26th from 4:30 - 10pm (Shores Arena)
Monday Sept. 27th between 5:30-8pm (Thedford Arena)
Tuesday Sept. 28th from 4:30 and 10pm (Shores Arena) (last time)

***You can email a copy of the vaccine certificate to [email protected].
 
 
Any of the above stated participants unable to provide proof of at least one vaccine, or letter of exemption, by Sept. 28th 2021, will be ineligible to play during the 2021-2022 season.  This is in keeping with the OMHA’s timeline to complete full vaccination by Oct. 31st.

The onus to prove a second vaccine status is on the parent to give to the registrar by October 31st, 2021.

On another note, the registrar will also be collecting the $75 rep fee at the door during these times.  Reminder, if your young player can’t dress independently, please ensure there is a caregiver present to help with gear and skates.

While we cannot help you make up your mind, and we cannot change OMHA's ruling, we did want to provide you with as much information as possible. With that in mind, here is our refund policy for the 2021-22 hockey season.

Refunds for any player withdrawing from LSMHA will be granted according to the date the written application to [email protected] is received by LSMHA and the following table: **Note, there will be a $50 admin fee charged automatically for any refunds. 
Up to September 30 - 90% of the remaining registration balance
During the month of October - 75% of the remaining registration balance
During the month of November - 50% of the remaining registration balance
During the month of December - 25% of the remaining registration balance
As of January 1 - 0% No late charges will be refunded.
