2021-2022 Registration

LSMHA is now accepting registration information for next season! 

Please fully complete the attached form for each child that you will be registering for the 2021/2022 Lambton Shores Minor Hockey Season. 

Please note some important information: all online registration forms completed in full prior to June 1, 2021 will be eligible for the regular registration rate.

Players that had their 2020 fees rolled over to the 2021/2022 season will be eligible for the early bird rate but still need to complete this form for each player.

Any returning player that does not complete the registration form prior to June 1st, will be required to pay the late registration fee as listed on lambtonshoresminorhockey.ca.

At this time, we will NOT be collecting registration fees. Registration fees will be due prior to the beginning of next season, and will be communicated once LSMHA gets approval from OMHA and the Municipality of Lambton Shores regarding COVID-19 and the start of the 2021 hockey season.
