2021 Registration - ONLINE
As a result of OMHA and Health Canada's rules and regulations regarding COVID-19, LSMHA has changed the way in which players will register for the 2020-2021 season.

All players are asked to complete the online registration.

The deadline for regular registration for the upcoming season is May 31, 2020.  We will not be collecting any fees at this time, but are requiring ALL players to register by completing the online form found at the bottom of this page.

Please note, the following registration fees.  We will be continuing with the volunteer cheques and the volunteer fundraising cheques this season as well.  All volunteer and fundraising cheques will be collected at the first tryout session for each player.

Please note, that while we are not collecting any funds at this time, every player MUST have paid in full, or be participating in a board approved payment schedule in order to be able to attend the first tryout.

Lastly, should our season be delayed due to Health Canada and OMHA's response to COVID-19,  LSMHA will reduce the registration fees for the 2020-2021 season and any player that has made a payment will receive a refund in the amount of the overpayment.

ADDITIONAL VOLUNTEER REQUIREMENTS PER FAMILY: In order for LSMHA to be successful we require the help of many volunteers, both on the ice and behind the scenes.

Each family is required to physically volunteer for at least 2 x 4 hour sessions throughout the season. Information and postings about volunteer opportunities will be available throughout the season on our website.

Each family will also have the opportunity to volunteer to fundraise for the Association. Volunteering to fundraise consists of the following.

* Families that indicate on the paper copy of the registration form that they agree to volunteer to sell tickets will be given a booklet(s) of tickets valued at $100 for each draw. * Tickets will be handed out by parent reps during the season * Families will sell the tickets and return the stubs to their parent rep * The funds collected for these ticket sales will be kept by the individual families to offset the cheque that will be cashed. * If tickets and money are not returned by the due date or a family declines to volunteer to sell the tickets the postdated cheque will be cashed.
